Family medical history?

Is there a website that I can find a form to fill out with my family medical history? Something that might be used in an adoption situation for the child in the future.

  1. Just type one up.  Think of anything that might of interest to a child.  A family history of diabetes, depression, heart disease, thyroid disease, seizures, any cancer and who, maybe your blood type, did you wear glasses, list any chronic conditions you had as a child, asthma, etc.  Really any information you are willing to share would be beneficial!


  2. If you have already placed a child for adoption or are planning to.  The agency will have these forms.  Once this information is compiled it needs to be placed with the adoption records because that is the first place someone will go to for information.  The other thing to remember is that our medical information changes as we get older and must be updated.

    Your Dr's office also has these forms or just do an internet search for medical history questionnaire.

  3. Not sure what state you are in, but your state may have a special form.  In PA, there is this one posted on the web, that I'm sure you could use for your own purposes.  Go to:

    I just typed in "family medical history form for adoption" into a web search and a bunch of things popped.  Perhaps you can find your own state that way.
