Fax transmittal forms?

Question

Ok so I'm trying to get a fax all set to go out but I have one problem... when using Works and filling the templet out where it asks for company name at the top do I put my name (sender) or the company I'm sending it to?

I have never done one before and have been looking around the internet for the answer and can't find anything.

Help PLEASE!

I have to have this out by 1pm today.

We made our own.

Just something simple that says FAX COVER SHEET

FROM your name, company name and your contact numbers

TO the person you are sending it to along with their contact numbers.

TOTAL PAGES, INCLUDING THIS PAGE

A place to write notes and you are done.
