Filling Standard Affidavit Annexure I for Indian passport application?

I need some clarifications for filling up the standard affidavit (Annexure I): https://passport.gov.in/cpv/ANNEXUREI_std.htm

1) Does non-judicial stamp paper mean a plain paper with revenue stamp on it?

2) Who is a "Notary Public" and how to get attestation from him/her?

3) End of the affidavit needs the signature of "Deponent". Is deponent the applicant or someone else?

  1. Non-Judicial Stamp Paper you can procured from any Stamp Vender sitting outside the District Court you reside, just ask the person for non-judicial stamp paper of the value Rs2/-,5/-,10/-,100/- as you may require, tell him your or the name of the person who has to give the affidavit on it, father’s or husband’s name & address. Secondly the ‘Notary Public’ is an advocate/lawyer sitting outside district court where you see most of the advocates/lawyers sitting; this fellow will put his notaral stamp & signature of attestation on the affidavit made on the NJ Stamp Paper, which is duly signed by the deponent or maker of the affidavit.He shall charge you for attestation of the affidavit. Deponent is the person who is supposed to give this affidavit & he has to put signature at two places, firstly on the first mark of deponent pertaining to main affidavit & again after the verification note of the given affidavit where again deponent is marked.


  2. just go to a court in your city. u will find tens of notaries in the court premises. contact one, he will get u a stamp paper, get your annexure ! typed out and notarise the paper. if u are applying for passport then u are the deponent. for the rest dont bother, just go the court, ctc a notary and get your jobe done by paying for his charges.

  3. Go to any civil court near you. You will get many sign board saying Notary Public. They r special Lawyers. Ask them that u want an affidavit for Passport. They will arrange all. The charges are 10-20 rupees+cost of paper, typing etc.

    Yes, u r the deponent. But don't worry, they will take care of all things.
