First Passport for my Baby..need help filling the form?

I had my first baby boy 3 weeks ago and I was just filling his passport application form and was stuck at few questions, could you pls help me as i am not feeling well and cant think or concentrate properly( still recovering from a terrible childbirth experience). Your help will be appreciated A LOT

1.town and county : FULHAM / LONDON

( my questionis: shall i put FULHAM under town and LONDON under county or the opposit)

2.it says in the form : if both parents were born abroad we will also need the full name , town , country, date of birth and date of marriage of your grandparents( or details of your parents' claim to british nationality) write this details in section 8, or on a separate sheet of paper. ( my question is:i was born abraod& i got my british nationality couple of years ago/ my husband is Algerian,how do i satisfy this part of the form?shall i send my certificate of british nationality& give details of my husband's parents?)
