  1. Your passport will cover you for ID.  The border agent will also ask you to fill out a form on why you are going to the USA.  One of the questions is the address of where you are going.  You could bring that with you on a scrap piece of paper.  We were asked that at the Canadian point of orgin, not when we got off the plane in the USA.

Also being your drivers license and health card.  If you got private health insurance you should also bring that card with you.  (the one they give you to call if something happens-sometimes it is a printout)  Make sure you GET that private health insurance if you arent covered by work...$60 is much better than thousands if something happens to you.  (ambulance alone is expensive down there!)

If you are bringing any iPods or other electronics with you, you could write that down with the products codes on them.  That way when you come back, they dont say you bought those in the USA.

  2. No,you don't need any other form of ID.

The passport is the only official document you need to enter the US.

Maybe canadian driver's licence if you want to drive in the US but it's not a requirement,it's up to you.

  3. All you need is a valid passport.

  4. Your passport is all you will need.  That will take care of it.  Of course if you want to drive in the US you also need to bring your driver's license.

  5. All you need is a valid passport
