A/Yes, it very could have. A bad system is a major problem no matter where you are, if anything in it isn't on par, things can only go wrong. An object is only as strong as its weakest point.>>

  1. Any object, can be defined in levels of strength. For example: A door that is well locked can keep out intruders. But the door itself is only as strong as the weakest point. (e.g. in most cases the hinges or weakest hinge) This also applies to buildings, vehicles, and the human body. (This is more than applicable to a submarine or the system, in an effort to computerize it.)
