  1. It is the forefinger (the one that points) that clamps down on the first two strings--E and B, not the index finger.

The index finger will go on the third string, second fret while the ring finger presses down on the fourth string, third fret.

An exception to this is when the thumb is used to catch the sixth string on the first fret. Then you would place the ring finger on the fifth string, third fret and the little finger directly beneath on the fourth string, third fret.

  2. middle finger is on the second fret 3rd string, ring finger is on the third fret 5th string, and the pinky is on the third fret 4th string

  3. Index finger you barre the B and high E string first fret, middle finger second fret G string, ring finger third fret D string.

  5. FYI to guitarpicker56 --

The terms "forefinger" and "index finger" are interchangeable terms for the pointer finger, the one closest to the thumb, aka the first finger.

What you erroneously call the index finger is the middle finger (usually the longest on most people's hands), aka the second finger.

Then comes the ring finger, aka the third finger. Then finally the pinky, which is the fourth finger.

So the F chord is played by partially barring across the high E and B strings at the first fret with the index (1st, or forefinger), then the middle (2nd) finger goes on the G string second fret, and the ring (3rd) finger goes on the D string third fret.
