For proof that i am a student to get discount gym membership..?

What does the teacher have to write down..

just that i am a full time student and a signature and date?

usually you have a student ID.

Just turn up at the gym looking like a fashion victim who just got out of bed at 2pm.  Push your way to the front of the queue before answering your mobile to talk c**p to another of your species whilst the receptionist and the increasingly annoyed line of people behind you wait impatiently for you to acknowledge that the world does not revolve around you.  That's all the proof they'll need that you're a student.

I never had to ever show any proof, but some places may require it, just bring your student card with you or your course enrolment letter.
