Form I-751 question?

Question

(1)Can required affidavit be one paragraph essay or does it have to be in some other form?

(2) Does it have to be notorized infront of any public notary?

(3)Also when I send a copy of our joint checking account, can we cross out the checking account number for identity theft safety or should we just leave it?

Affidavit must be in the required form and it must be notarized.  The checking account must not be altered so that the application will be approved.
