Form in Doc format (MS Word) ?

I want create a form in ms word where nobody can change the headings they use fill the form.

not edit option in headings who is filling the form.

  1. dude the best protection technique to provide documentation is to use protect document from tools menu else the other option is to apply permissions to the file and assign the email address which can be referred to it...

    This permissions option is found under file menu and you can apply restriction to the file and the users whether it is Read mode or write mode??

  2. This works only on Office 2003 or later versions:

    After you are done designing the form, go to Tools >> Protect Document.

    The Protect Document task pane appears.

    In the second drop-down box, select "Filling in Forms".

    Click on the "Enforce Protection" button, and set a password.

    Save the document.
