French final HELP?

k, well my french final is on monday (the oral part)

this is wat my french teacher said we need to talk about:

U are ill and must include the following info:

1. wat is ur name (present tense)

2. wat is ur a age (present tense)

3. wat r ur symtoms(present tense)

4. did ur mom or dad take u to the doctor? (passe compose)

5. did the doctor describe any med., if so wat kind? (passe compose)

6. When will u start to feel better? (near future)

7. Do u want visitors?(present)

8. When will u be able to continue w. ur normal routine? (near future)

this is wat i got so far:

1. Je m'appelle Sitara.

2. J'ai quatorze ans.

3. Je suis en mauvais sant'e. J'ai mal au ventre. (i have a bad stomache)

4. Mon pere ne me m'a amenee pas au hopital, mais il me m'a amene au clinic.

5. LE docteur ne me donne pas une medicine.

6/ Je pense, je vais sens miex a prochain lundi.

7. Je ne veux pas les visiters

8. ?????

4-mon pére ne m'a pas amené à l'hopital, mais il m'a amené au clinic.

5-le docteur ne m'a pas donné des médicaments.

6-je pense que je me sente mieux Lundi prochain.

7-je ne veux pas que quelqu'un me rendre visite.

8-je pourrai reprondre ma vie cotidienne aprés une semaine.(i will be able to continue my normal routine after one week)

" ne me m'a amenee "-(delete "me"). The rest looks pretty good. GOOD LUCK!
