GL Penpal form?

I wanted to get a penpal from Girls Life Magazine, and when I went onto the website, I could not find the form that I needed to fill out. Does anyone have the link, or could anyone tell me where to find the form?

if you have one of the magazines look at the front where it talks about the penpals and then it will say where to go. It costs money.
