  1. you will need to scrape off the top soil and add shale...then 4 in. of crushed limestone and 6 in. of concrete...want a nice slab for your garage...hire it out...

  2. I would use a medium sized stone of about 3/4" in size and lay a base of at least 6 inches of stone then pour a cement slab of 5 inches in depth.  A 5 or 6 bag mix (per yard).  call an excavator they will haul in the stone by the truck load and bring you the proper stone.

  3. You need to put a layer of hardcore down first as a base. Then you'll need bags of cement and sharp sand for the top layer. The best thing to do is go to a builder's merchant, taking your measurements. They will be able to advise you and sell you the exact quantity of materials you will need. It would be best for you to hire a cement mixer too. It might be cheaper to let a professional do it.

  4. Crushed lime or lime dust is the best.

It requires no compacting and is so dense there is virtually no settleing.

  5. Best would be to use road base, generally 3/8 and less gravel compacted in place.
