Getting a pony card...?

Question

I have been through USEF and cant find how to get a permenate height card for my pony; Can some one tell me how to do this and/or a link to the forms and rules. Thanks!

To obtain your measurement card, a steward and vet at a show where your pony is entered to compete must take the measurement. The link to the form is:

As a side note, I worked for big pony trainer at one point. His ponies were top finishers at Pony Finals every year. Well, you would not believe some of the theatrics that went on for measurements. Some trainers would pull their ponies shoes off or shave down their feet, trying to lose an inch or two to put them in a lower division. Some would stand the ponies on grass and hope the steward wouldn't notice (the measurement's supposed to be taken on paved ground). It was really insane.
