Getting closure in a relationship?

What are some good questions to ask? I know only we know what went on in our relationship but what are some general questions or good things to say or point out when getting closure. I don't want to leave anything out because I want this to be the last time I talk to him.

nothing just tell the other party frankly and the reasons. Guy hate to beat around the bush when it come to this kind of thing. just tell him straight and maybe you could continue to be friend.
