Going from Playa del Carmen to Belize (City)?

...how much trouble would that be in terms of means of transportation, safety and health issues (malaria)?

You can take an ADO bus from Playa del Carmen to Chetumal.  These are very safe, nice buses.  There are 21 per day on that run and the fare is 190 pesos...about $19.00 USD.  See 1st site below.  Getting info from Chetumal to Belize is harder.  See 2nd site and scroll down to the photo of Novelo's bus station.  Now look immediately above the photo and read everything in the box. When you buy your ADO ticket, they may be able to tell you something about connections in Chetumal to Belize..and then, they may not.  Might have to get that at the bus station in Chetumal .  Hope all works out well for you.

Take mosquito repellent and malaria shouldn't be a problem.  

If I were taking this trip, I would take the bus from PDC to Chetumal and sight see around Chetumal the rest of the day.  After checking into your hotel, I would call Challenger Taxi to pick you up the next morning at your hotel.  They will get you through customs and take you anywhere in Belize you like.  But  you could just ask him to drop you at the Corazal Town bus station.  From there you can bus to Belize City.

Belize City is NOT a Caribbean paradise and for safety, do not wander around at night--Ambergris Caye is safer and nicer.

I would not go there Belize is a dangerous place and Malaria you can get that easily and people get killed there like everyday my advice is to pass ur time in Cancun cuz I live in Belize Its pretty boring & very dangerous
