Gotta do community service

Question

18 hours i think, and the judge gave me a paper but i dont no how to get the hours written down on a paper, like a certificet from my church/ hospital, they never gave me such thing to write down the date and the signature and hours from doing community service,, do i jsut make my own paper and write hours and get signs???

WEN I HAD COURT HOURS... WHEREVER I DID MY HOURS...I HAD TO RECEIVE A LETTER SIGNED AND DATED WITH HOWEVER MANY HOURS I DID

yes. I had that for NJHS. There was a paper but sometimes people lost it so they used a regular sheet of paper. Just use that.
