HELP!!! I lost my birth certificate years ago. Do I call the hospital I was born in?

I'm in Chicago and I was born in St. Louis. What info should I have ready? What's the quickest way to go about doing this? Thanks!!!

  1. Either drive down to St. Louis Co on Monday and get a copy at the county building or send them a letter with a copy of your driver's license and they'll mail it back to you. The hospital doesn't keep your birth certificate. They just file it with the county after you're discharged. Hospitals do not provide official birth certificates.  The one provided at birth is not official.  The official one has to be acquired from the Dept. of Vital Statistics of the state you were born in.  Pretty easy to get and the fee is around $12.  Do a internet search for the number for the Dept. of Vital Statistics for Missouri and call them.

  3. Call the county clerk's office in which the hospital is located in St. Louis.  If you are a minor, you will need your parents to call.  The county clerk can then tell you the best way to proceed.

  4. you can contact the county courthouse where you were born and the last i checked the cost was 10.00

  5. you usually just go to city hall and apply for a new copy. take your social security card and a photo id with you, theyll need it. in ohio, a replacement birth certificate is about $18.
