  1. Try the last school you attended, your family doctor's office, or your parents.  If that doesn't work talk to your local health department and see if they have any suggestions.


  2. I couldn't find mine, so they took blood and tested my immunity for different things.  Another girl I work with called her high school (she's 30 years old) and they still had them.

  3. You need to contact all the doctors from your past and get the immunization records from them. You'll need to fill out forms saying you want those records released to you.

    Also, it may take a bit, so I would get started.

  4. Your old school districts should have most of what you need.

  5. I too needed them for my Nursing job, contact your GP he'll be happy to provide a letter stating what you have had done and the dates. It's the same for the US as the UK.

  6. Your doctor will have the details.  Call the surgery, some nice receptionist might print out the info for you!
