  1. If it's on the first floor, locate the wall in the basement, this is easy if there is an electrical outlet on that wall.  Drill up into the wall.  Cut a hole in the wall directly above the hole you drilled and have someone push the wire up from the basement.  It takes some time and fishing around but you'll get it.  This assumes itis an inside wall.  I wouldn't mess with an exterior wall and s***w up the insulation

If it's on the second floor, you hope there is a closet underneth the wall, otherwise, you gotta start ripping things up

  2. If you are running a wire from the attic down to the TV you just go in the attic and drill a hole in the top of the wall and drop the wire down to the TV.   If you are running wire from the TV down to the Cable box or other componets you just cut a hole near the TV and drop them down to a hole near the componets.  You have to run the wires up and down between the studs.  It is much more difficult to run wire horizontal in the wall.

  3. Interior walls are best because you won't run into insulation or the low pitch of the roof that you may encounter with exterior walls.  Between the studs is a whole open area - but you have to drill down from the top plate by getting up in the attic craw space and drilling in the right spot. That is if you want to do all the wire runs in the ceiling and just drop down and cut through the wall where you want them to exit. It will be near impossible to run the wires horizontally behind the walls because you will keep running into studs every 18 -24 inches. So up and over is the best bet. You can "fish" wires in the wall by running a stiff wire - like a coat hanger - with a hook on the end to help you catch it from the other end and pull your wires up through that space.
