HW help: how do you annotate?

I need help annotating this essay my teacher gave me. What's a simple way/format of doing this?

One way is to use the margins. Next to the points you want to comment on write a note in the margin.

e.g

Body                                                                        Margin

In the play Macbeth by      - Macbeth should be underlined

William Shakespeare...     -Clearly and concisely introduces book,
