  1. Check the site "Know your PAN" and you may get some details.

    Take a print out.

Approach Help Desk at NSDL or UTI with all the evediences you have and request them to sort out the issues.

Change in address has to be supported by a proof as per the norms.

Letter from UTI with 3 nos. is not understood.

Hence approach the office of UTI from where the letter originated and request them to assist and clarify steps essential for your mother to receive a PAN card with the no, allotted to her.

While checking the PAN details on the site, ensure that there is no spelling mistake - as you are supposed to mention the alphabet letters in the name as was referred by at the time of your application.

You may not have retained the duplicate copy of your first initial application for PAN.

Call center attached for PAN queries are also helpful and you will have to be patient, with timely follow ups, essential for the solutions.

  2. The correct name as per the tax records appears to be VEENA SHREEPATHI KARANTH.

The pan NO is correct.  Please submit a fresh application with proof of residence, date of birth and correct identification.  The related form is available on the website of UTITSL or NSDL.co.in.  Please fill in the application and submit alongwith three passport size photographs to UTITSL.  While applying give reference numbers as indicated by you.  A fee of Rs. 60 or Rs. 70 will also be required to be paid.  You can pay this either by a credit card or by sending a DD.
