    i never had a form to leave the usa.

  2. If the airline forgot to take your I-94 upon departure from the U.S. you can have problems upon trying to go back to the U.S.  You should mail your I-94 back to Customs and Border Patrol. They will also want to see your boarding passes and copies of entry and exit stamps from your passport.   3. Yes, it happens.  Don't be shocked if your departure is not documented and you get listed as having overstayed.  You'd better keep that airline ticket and passport as proof you left on time.
