Has anyone ever placed a tape recorder on a grave?

No, but I once placed my crystal on the headstone of a civil war soldier.  The thing lite up like a flare, and the hum was defening.

I've seen a number of websites with EVP phenomena recorded inside graveyards.  Sometimes disembodied voices speak, sometimes there are dreadful screams and roars.  Search the web and I'm sure you'll find plenty of examples.

yes...you have to pick a grave that is older than 25 years,has no religious symbols on the tombstone,needs to be a child or young adult,leave at least an hour,secluded graveyard with no traffic noise...it works...you hear some weird stuff on it for sure....babies crying,voices mumbling etc....scary...
