Has anyone had to transfer from college to another college closer to home because of medical reasons?

im a junior but i am going through a lot medically and there is a chance that i will have to transfer. the only problem is that it will set me back considerably as far credit hour.

Are you sure about this?  Have you checked with the other college to see how many hours they will accept from your current college.  Either way, it may be worth it to be closer to home where you have a support system while going through what you are going through.
