Have there been any recent (human) deaths in slaughterhouses?

I know that question is totally weird but I'm trying to write an essay for school about Fast Food Nation, so I was wondering if anyone knew of a news story related to a death in a slaughterhouse or meat processing plant due to safety negligence.
