Yes.  Deepest was >180 feet--stupid but I did it and live to say I did it.  Now I typically like to stay around the 40-60 foot depth.  It gets dark and cold quickly here.

Yes, still do since it's both how I earn a living and what I do for recreation.

Yes, and it was by accident too. The lead guy told us to stay in near a field. And the field was huge. HUGE. So i didn't realize until when I drifted off to about 160ft. Really dark there. Scary too, and not exactly welcoming.
