Header for Microsoft Office?

Question

How do you insert your last name and page number without it being the same on every page? Ex: Smith 2 in the header on every page. How do I avoid this and have the numbers different?

It depends on which version of office you have, but what you're looking for is to insert the page number.  Look around for it in insert (I think).  Sorry, I don't have it open right now.
