  1. The policy number is what uniquely identifies you (and your dependents) to the insurance company.

    The group number is what uniquely identifies the type of plan that you have to the insurance company.  (A "group number" is often specific to your employer and their benefit plan.  A group number usually has multiple policy numbers associated with it...for example, you and all your co-workers on the benefit plan.)

    Group and policy numbers are not the same thing. Only your policy number uniquely identifies you/your family, and your medical provider won't be able to bill the insurance company without it.

  3. It's the reference number given to your file within the insurance agency.  Referencing your policy number is the only way the insurance company will be able to understand what is covered and what isn't.

    It's usually listed at the top of your billing statement or insurance card.  It might also be called a "group" number.
