Health insurance policy number...what the heck does that mean?

I was filling out a medical history sheet and it asked me my health insurance company (i know that..) and then it asked me what my policy#: was...

The policy number is what uniquely identifies you (and your dependents) to the insurance company.

The group number is what uniquely identifies the type of plan that you have to the insurance company.  (A "group number" is often specific to your employer and their benefit plan.  A group number usually has multiple policy numbers associated with it...for example, you and all your co-workers on the benefit plan.)

Group and policy numbers are not the same thing. Only your policy number uniquely identifies you/your family, and your medical provider won't be able to bill the insurance company without it.

It's the reference number given to your file within the insurance agency.  Referencing your policy number is the only way the insurance company will be able to understand what is covered and what isn't.

It's usually listed at the top of your billing statement or insurance card.  It might also be called a "group" number.

They want the numbers from your health insurance card. It might be called "policy number" or "group number" or something like that. They need some way to identify exactly what type of insurance you have and what group it's through.

(Usually the best way to deal with these kinds of forms is just to have a sheet of paper with a photocopy of the front and the back of your card -- then when they ask for all the numbers you can just write "see attached" and hand them that paper along with the form.)
