Do I need those requieremnets before I can travel on August to the Philippines bringing my 5months old daughther along with me? and please help me what are the other requirements do I need to prepare.I will travelling with my daughter only and do I need to get tavel authorization from my husband?and what family do I use to get airplane ticket?and do I need to have new passport since my Philippines passport will expire on 2011?Please help me.

you don't need to file these papers unless you want your baby to become a philippine citizen (report of birth) or want to renew your passport and have it carry your married name (report of marriage).

if your baby is staying in the philippines for 21 days or less, there's no need for a visa. if she stays more than 21 days, then a visa is needed... unless she's a philippine citizen also (then the report of birth is needed to be filed).

all you need to bring your daughter to the philippines is her passport and a notarized affidavit from the father stating that he is allowing you to bring your daughter to the philippines for a visit.

there's no need to get a new passport for you since it will not expire in 3 years.
