Dear Sir,

After my accident , I felt terrible pain in my heap joint in the morning.

I had high uric acid. Its increase and decrease. Moreover, from last month I face teeth problem. I am under medication.

Yesterday, I did my scaling.

And it pain and bleed a lot.So, I become late today.

And I believe I am professional. From tomorrow.

Tanks for your friendly reminder.

Please note; our scanning machine doesn’t work so , I send you’re the photocopy of the medical paper by hand.

Dear Sir,

The morning after my accident I suffered from terrible hip pain.

I had high levels of uric acid which fluctuated. Additionally, for the last month I have suffered from dental problems for which I am currently taking medication.

Yesterday, I had a scaling procedure done. As a result of the painful procedure I was late today. Regardless of the pain I will remain professional and dedicated. Unfortunately our scanner is not working, but I will be certain to deliver my medical documentation in person.

Post-accident mornings I began encountering a terrible pain which originates on the lateral portion of my pelvis (after hip write if it&#039;s on your left or right side).  Moreover, beginning last month I began experiencing dental difficulty.
