Help unlocking samsung t809?

I followed the codes on web and i unlocked my phone but when i turn it off and turn it back on it got locked again...but when i enter *7465625*638*Unlocked Code# its tells me already personalized..anybody with help thanks.....

It sounds to me that it's not an unlock code for your sim card, rather just a security unlock code for your phone in general. This is something that is set up by the user (unless it's the PUK code that you need, then you'll just need to ask for the code from your service provider who I think should be T-Mobile). Either that, or you could try the default codes that I'm giving to you:
