Help with autobiography........

For acceptance in the nursing program at my college I have to write and autobiography, and I need some help. Below is what is stated:

(Autobiography (Personal history, education, work experience, influences on the decision to pursue a health occupations career) I have never done one before and any advice would be helpful..thanks.

Hmm, strange, my brothers GF had too do the same thing.

She had made up some lies - these people don't know everything about you.

Start off with where you were born, then, tell them how you acted.

Tell them about your family - how many member are in it, if you have any pets -if you WANT too get any pets.

Then- tell them how your first years of school were.

Tell them if you participated in anything in school - i.e. , A volleyball team (This is where she lied).

After that, tell them about your high school years. Tell them why you liked it or not.

Lastly, just speak about where you are now - and your perspective of the future for you.

Write it in paragraph form; and put each topic in its own paragraph.  Start off by introducing yourself (introductory paragraph) and give some personal details about who you are, where you come from, siblings, children, etc.  This will cover "personal history".  The next paragraph will be all about your education.  Etc.
