  1. your balance is off. dont lean as back as far ur not pancho sulliven.and put more weight on your back foot and have some one video tape you

  2. The other answerers are correct; i.e get someone to video you and you could see why this is happening.

If you can't get a video for some reason, here are some reasons why this may be happening: 1) The tail portion of your board may be too wide and/or your fins are too small; or 2) You are doing your cutback on the wrong part of the wave (either too far on the shoulder or too close to the curl); or 3) Your bottom turn is either too weak where you didn't generate enough speed so when you want to cutback you are just kicking your board out of the wave; or your bottom turn is too hard where you have generated too much speed and your board is tracking to the point that breaking its track will cause your fins to pop out.  If this is the case, remember that a cutback is not a move by itself; instead it is either part of a series of moves like a shortstop catching a ground ball and throwing to first base in one smooth motion.

Hope you find the answer to your problem.  In the meantime just keep in mind that a cutback is not mastered overnight.  It will take a lot of practice to perfect... so keep trying.
