  2. If you are under 18, you will need both parents signatures on any name change.

  3. Yes .Go to your  local county courthouse and they will inform you as to what to do.

  4. yes, but you have to go to court and have a legitimate reason for wanting to change it. for example, a stage name or whatever. I knew this guy who was from another country (back when I was in college I knew a lot of exchange students but this guy had decided he wasn't returning-that he was staying in America) and his surname was like 30 letters long so he wanted to shorten it. I think back then it cost him about 200 dollars so my guess is now it would probably cost about 500 dollars cos that was when I was in my first year of college back east-way back in 1976-so I know the inflation rate has gone up some and also remember that you must have it all writtten out the reason you want to change it: for you it is simple: you want your surname restored. But write it down for the judge and as long as the judge feels it is a legitimate reason; he will allow you to change your surname. Atleast this is the way it was back in 1976. And the guy was a good friend of mine so I remember a lot about it.

  6. Yes, you can change back to your original surname. This is called a "Deed Poll". Its real expensive in Jamaica, i am not sure about ur country. Good luck!!

  7. Depends on your age.  Look up the number of your local Family and Probate Court and call for information.  They can tell you how old you have to be to file a name change petition in your state, and what fee is involved.  (It probably won't cost much.)
