Hi all, anyone have any sites where I can get a template for a pre-school exit/feedback form .?

  1. There's a good book on amazon.co.uk called 'early years manager' or something like that which has loads of templates...not sure if that'll help if its by Friday but its a handy book anyway.

    Otherwise just do it in the form of a questionnaire-thats what we use:

    -Do u feel you and your child enjoyed pre-school?

    -Did you feel that we communicated effectively and gave you useful information?

    -Please tick which was useful to you: Diary, key person, newsletters....etc

    -Do you feel your child was-and then i listed the Every Child MAtters headings and parents ticked them.

    -Would you recommend us to a friend/family, would you send younger siblings to us?

    Those sort of questions really...then a comment box at the end...we gave them out in plain white envelopes and had a sealed box for replies-this way it was total anonmous!

  2. Bittersweet is right, try developing your own as this way it is personal. sHe has suggested some good ideas for questions to ask so not much need for me to add here.
