"Life is short, and the Art long; the occasion fleeting; experience fallacious, and judgment difficult. The physician must not only be prepared to do what is right himself, but also to make the patient, the attendants, and externals cooperate."

can someone please explain to me what this actually means

  1. The Hippocratic quote you referenced relates to the ongoing "practice" of medicine.  One may have moments of brilliance when diagnosing/treating the ill, but these experiences can be fleeting. The practice involves a lifelong dedication to the art of treatment and prevention.  You will fail, even with the best of intentions.  A good doctor should never succumb to megalomania -- each judgment regarding a patient's care should not involve hubris.  Judge each treatment as you would want to be treated.  Do not let emotion or influence of others deter you from doing what you have determined to be in the best interest of the patient.
