Xtreme HD 4x1 sold by Apple is $99.

Make sure you get one with remote, so you can use universal remote.

The Yamaha RX-Z11 has 5 HDMI inputs and 2 outputs....but it costs like 5,499$. The upside is that the receiver will upconvert to 1080p any video source even composite, s-video and component.....switcher's are much much cheaper but no video up-conversion.

It doesn't matter -- cable formats seem to change every 3-5 years.
