Housebreaking a puppy?

My boyfriend and I just moved into a new apartment and we have a four month old puppy. She was completely house broken in my other house but now she is having a lot of accidents. I want to break her of the wee wee pads so I moved the wee wee pad out onto the deck but she wouldnt go on it. I brought it back in and she peed on it multiple times but last night she peed on my comforter and on my bathroom mat. Seeing as she has used the wee wee pads in this apartment, she obviously knows where she is supposed to go. Walks are a hit or miss. Sometimes she will completely relieve herself on a walk, other times she will hold one or both for the wee wee pad as soon as we get in. I'm not sure how to handle this situation if someone could give me some input.
