It's really pretty simple and Bataman answered it in the movie.  Ask yourself this, who would be crazy enough to follow the Joker since he's obviously Brilliant but totally insane?  The answer is someone who's just a crazy as he is.  They were all initially Arkham Asylum escapees.   He later recruited real criminals when they realized he may be out of his mind but he's damned good.
