The short answers is, if you play Vampire the Masquerade (VtM) you are most likely start play already as Vampire. The much longer and technical answer is when a Vampire killed you (your mortal self) by drinking all your blood. A single drop of Vampire blood may have been transferred to you to very moment you died. This might have been planed or a complete accident by the "Sire" Vampire. Anyhow when you wake, you might find you self completely psychology or physically changed. This would be the start of your new "Unlife" you are now a Vampire.
