How and when did the no doc loan policy originate?

Don't know when and just guessing on the how.

Those fortunate enough to be self employed and stable need loans as well.  Unfortunately, the standard docs do not apply thus the originators had to use other forms of verification (Credit Score) to take a guess at credit worthiness.
