How are phone card numbers generated?

how come they are never duplicated and I cant just guess a number which works

  1. The many companies purchase numbers in blocks. When numbers expire they are moved to the back of the block sometimes it takes years if ever to be re-used. This has forced the industry to institute more area codes. Because most numeric combinations have been used up.One can guess but the area code is the most important differing factor.
