  1. at first you have to choose the college, you can get free application form, from http://directdegree.info/

  2. I can't tell you specifically re: Scotland, because I'm not familiar enough with the system there to be able to translate from US to Scotland. But I can give you approximate English equivalents, which maybe will serve, and then you can do the conversion for Scotland?

Vassar's a competitive school. Most US students accepted have a GPA of around 3.7 out of 4.0 - that's mostly As, maybe some A-s or a B or two. In England, that'd mean doing very well on GCSEs, and then it would be quite helpful if she had A levels of around AAA or AABish. Roughly, because there's no exact equivalent. But you can email Vassar directly to ask them what she needs from Scotland. They will know, and will be able to advise. I'll link, below.

Your daughter will also have to take the SAT exam, which she can do in the UK. She'll take the main SAT, plus two SAT subject tests. SAT subject tests are vaguely like A levels, sort of.

The other thing that US universities take into account, which isn't part of the admissions process in most of the UK, is extra curricular activities. They look for her to have done things in school and/or outside it, apart from her classwork, such as having had a part-time job, or doing volunteer work, doing sports at school, or being part of the math club - anything, really, is okay, so long as she's shown some devotion to it.
