  1. You can do it using VoIP.

CallCentric is one of my VoIP service providers.  I have the ability to permanently block my caller-ID through my web account Dash Board administration page.  It is just one of the many free features included in their pay-as-you-go service.

  2. If you're not using a touchtone phone, you can push 1167.  Otherwise you'll have to contact your phone company to have your listing permanantly show up as 'Private'.

  3. 727 729 3253

  4. Contact your phone company to have them set your number to be withheld by default.

In most cases you can then release your number on a per-call basis if you wish to for a specific call, typically by dialing *82 first (in the U.S.).
