  2. You cannot. Medicaid is state specific so if you move to a different state you will have to re-apply in your new state. The qualifications with each state are different so you'll want to do your research to make sure you will qualify in the new state before you move.

  3. You can't.  Medicaid is run by each state.  If you move, you need to reapply to your new state of residence.  It doesn't transfer.

  4. You need to call your Medicaid provider and do a change of address.  If you don't know your provider's phone #, you can call the state office and get it through them.  If you don't have your state office number, you can find that online by doing a search for your state or city.
