Call your employer, they may have a copy.  I think you can call the Irs to get copies but it takes longer.

Your employer has that info, or go on irs.gov.

You should contact the employer if possible.  Wage transcripts from irs don't contain any state information but do have the name and address of the employer so you can contact them.  As a last resort you can get a copy of the w-2 from social security but they will charge a fee. Hope this helps
