How can I find the "unclaimed dividends" form from my broker?

I am with Scottrade. Should I just go to local office and ask for one?

  1. No, not sure where you are going with "unclaimed"

    The dividends should be in your account.  If the account is still open there are no "unclaimed dividends"

    If you think you are entitled to a dividend, and didn't receive it, bring it to the attention of the firm and they will research it for you.  If you were entitled to the dividend, the firm will credit your account and claim the street side.

    Unclaimed monies (dividends and/or balances) are from accounts where the owner of the account can not be found.

    These monies are echeted to the state of domicile of the last known address for the customer.

    If you think you're such a customer, you have to claim it from the state.  (don't bother the broker with this, no sales rep will know what to do or who to ask)  This can be done on line with the state, they will provide you with the necessary forms.

I think you might have to do more than ask.  What you need to do is check each monthly statement against the dividend payment schedule for each stock you own and make sure that you received the dividend and that it was for the correct amount.  If you did not receive the dividend or it was for the incorrect amount then you have a case for going to the broker and requesting a correction.   This can sometimes happen if you purchase stock close to the ex dividend date but in general it should be a rare occuance.
