How can I get a birth certificate online from jamaica?

I am from Jamaica and lost my birth certificate. I need to get one asap....Can someone help me????

you can apply online but you will need to know have these at hand to fill it out Full and correct name of child           Parish of birth

Child's Gender- male or female       Mother’s name

Child's Date of birth      Father’s name

Place of birth: name of hospital or address of home    Birth entry number

District of birth      Place of registration
