  1. You or your brother must pay the SEVIS fee, then he can apply for his student visa at the nearest US embassy or consulate.  Here is more information:

  2. The school should have sent information on what to do.  First he'll need to pay his SEVIS fee (once he has the I-20 in hand).  Then, he'll need to go to a visa interview.  He'll have to "prove" that he's planning to come home (show that he has family at home, a job waiting for him, car payments, house payments, etc.).  He'll also show that he can pay for his education.

    If you are already in the US, it may actually hurt his chances because they might assume that he won't come back.  He shouldn't lie about you being there.

  3. Your brother should first, contact  and submit applications to one or more universities, take TOEFL/ GRE tests.

    Based on his tests, interviews/acceptance he will receive the necessary papers for his visa. Only if he is accepted!!!

    Here are some excerpts from a governmental website.

    <<All applicants should be prepared to provide:

        * Transcripts and diplomas from previous institutions attended;

        * scores from standardized tests required by the educational institution such as the TOEFL, SAT, GRE, GMAT, etc.;

        * financial evidence that shows you or your parents who are sponsoring you have sufficient funds to cover your tuition and living expenses during the period of your intended study. For example, if you or your sponsor is a salaried employee, please bring income tax documents and original bank books and/or statements. If you or your sponsor own a business, please bring business registration, licenses, etc., and tax documents, as well as original bank books and/or statements.>>

    Good luck
